Each patient also teaches me so much. Grateful to be able to help facilitate people in their careers to be more independent, pain-free, and successful.

    services

  • As of January 1st 2022, Lifestyle OT will be a nonparticipating provider for insurance companies. Patients will receive a superbill for each session that can be submitted to their insurance for reimbursement.  Due to the challenges of running a small practice and transparency, this change was essential to keep serving the public as a value based practice.

If you plan to use your insurance to cover OT services, it is important that you first check that your insurance policy includes coverage for occupational therapy. Lifestyle OT is not responsible for insurance payments and cannot guarantee reimbursement.

Before our initial consultation patient is responsible to get:

    • A doctor referral with a diagnosis

    • Paperwork filled out prior to your visit

    • Confirmed insurance authorization before you visit.

    • Patient will submit Superbill to Insurance

  • We accept all major credit cards and cash and we can arrange mobile and online payments.

  • The health and safety of my clients is my top priority and I am doing all I can to protect you.

    This includes:

    • Automated scheduling and payment and paperless contact to let you know what is happening. Communication via text with updates

    • Office seating in the waiting room is physically distanced for therapy and evaluation to encourage physical distancing

    • All patients are asked to sign an informed consent form before coming to the session

    • Improved ventilation with air purification

    • I wear a mask with safety goggles. All patients are asked to wear mask as well

    • Appointments are scheduled to minimize having too many people in the waiting room

If any cases of Covid19 are discovered, clients will be notified
